Before seeing one of the chiropractic doctors at Back2Health, you will need to fill out a few forms.  Please print these forms so you can take a few minutes to fill them out prior to your first visit.   We are very detail-oriented at Back2Health, and want to make sure we have your complete health history so that our treatment and recommendations can be as effective as possible.

Loose clothing is best for your first appointment.  Most patients are comfortable in what they would typically work out in.  So, a t-shirt and shorts will do fine.  If you are a runner, please bring the shoes you’ve been running in. 

We are located at #450 – 2184 West Broadway in the Regent Medical Building.  This is between Arbutus and Yew, on the south side of Broadway View Map

Pay parking is available in the lot behind the building.  Metered parking is also available on surrounding streets.  Non-metered parking is available on the north side of Broadway or on adjacent streets.  Please allow yourself time to find parking when planning for your visit.
